We deal with the prevention, diagnosis and treatment of work-related conditions, with the aim of protecting workers' health and ensuring safety in the workplace. Occupational medicine

Company obligations?
Competent doctor and fitness, explained

Who must appoint a competent or authorised physician, how visits work and what the fitness judgement means.

Who is the Authorised Physician and how does it differ from the Competent Physician?

The Authorised Physician (Medico Autorizzato) has ultra-specialist training and experience, has passed a State examination and is entered in the register held at the Italian Ministry of Labour. While the Competent Physician (Medico Competente) handles health surveillance for general occupational risks (display screens, manual handling of loads), the Authorised Physician is the only figure entitled by law to oversee the health of workers exposed to ionising radiation, classified in Category A and B under Legislative Decree D.Lgs. 101/2020.

Which facilities are required to appoint an Authorised Physician?

The obligation applies wherever X-ray equipment or radioactive substances are present: dental practices (panoramic, Cone Beam, intraoral X-rays), radiology and diagnostic imaging clinics, veterinary clinics with an X-ray department and industrial laboratories using X-rays. In these cases, to issue the specific fitness-for-work certificates, the employer must appoint an Authorised Physician and cannot rely on the Competent Physician alone.

What additional examinations does the Authorised Physician carry out for radiation-exposed workers?

In addition to the medical examination focused on the organs targeted by radiation (skin, eye lens, haematopoietic system), the protocol includes targeted tests such as a complete blood count with differential and any eye examinations to prevent radiation cataracts. Thanks to the in-house Nursing Clinic, the exposed worker can carry out these tests in a single session.

Our dedicated health surveillance software allows companies to monitor deadlines, employee fitness certificates and documentation in a simple, effective way.

Fitness-for-work assessment

The occupational physician assesses employees’ health in relation to their duties and issues a fitness-for-work judgement, classified as:

Fit for work

the worker can carry out their duties without restrictions.

Partially fit

temporary or permanent limitations with specific prescriptions.

Temporarily unfit

the worker cannot carry out their duties for a defined period.

Permanently unfit

the worker is unable to carry out the assigned duties.

Medical examinations for health surveillance

Medical examinations are essential to protect workers’ health. They can be:

Pre-employment

before hiring or in the event of a change of duties.

Periodic

to monitor workers' health over time.

Extraordinary

at the request of the worker or the employer in the event of changes in the worker's health.

Authorised Physician for ionising radiation

Authorised Physician

The Authorised Physician (Medico Autorizzato) is a healthcare professional accredited by the Italian Ministry of Labour and Social Policies to carry out health surveillance of workers exposed to ionising radiation, in accordance with Legislative Decree D.Lgs. 101/2020 (the Italian regulation on radiation protection). An authorised physician is mandatory for all companies whose workers are exposed to ionising radiation.

Occupational medicine:
specialties serving the workforce

Occupational medicine combines health surveillance with the centre’s specialist visits, working alongside the company physician according to the risks of each role.

Occupational cardiology

Visit with occupational history, resting and exercise ECG, echocardiogram and Holter monitoring: for physically demanding roles, long shifts or cardiovascular risk.

Occupational ENT

Audiometric and impedance testing for workers exposed to noise; assessments for snoring and sleep apnoea.

Occupational pulmonology

Simple or global spirometry and DLCO for workers exposed to dust, fumes and gases, or with suspected respiratory disorders.

Neurology and neuropsychology

Assessment of attention, readiness and reflexes for surveillance and driving roles; neurocognitive screening where indicated.

Optometry and visual assessment

Visual analysis and tests for eye strain, acuity and binocular coordination: especially for video terminal workers over 20 hours a week.

Occupational podiatry and orthopaedics

Postural and baropodometric examination for prolonged standing, manual handling and long-distance driving.

Occupational dermatology

Visits for contact dermatitis, eczema and hypersensitivity in workers exposed to irritant or allergenic substances.

Testing, toxicology and fitness to drive

The in-house blood collection point performs the tests required by health surveillance, in the same session as the visit.

Blood tests for occupational risk

Basic or risk-specific check-ups (e.g. lead, ferritin, blood count, glucose, liver and kidney markers).

Toxicology screening

Alcohol and drug testing for the roles covered by the Italian State-Regions Agreement of 30/10/2007 (drivers, crane operators, safety-critical roles).

Obstructive sleep apnoea screening (OSAS)

Apnoea-related sleepiness is a recognised risk factor for workplace accidents and driving safety: sleep screening and, where indicated, polysomnography for drivers and shift workers.

Work-related stress
and workplace wellbeing

Work-related stress assessment is a legal requirement under art. 28 of Italian Legislative Decree 81/08. The centre supports companies beyond mere compliance:

Work-related stress assessment

Support for the employer and safety officer in the assessment required by the company risk document.

Individual and group psychological sessions

Programmes for managing stress, anxiety and burnout with the centre’s psychologists.

Exercise at the workplace

Guided active breaks, walking groups and postural programmes for desk-based and static roles, supervised by sports physicians and physiotherapists.

Smoking prevention

Periodic spirometry, motivational counselling and cessation programmes with pulmonology and psychology support.

Training at your company or at our clinics

Training and refresher courses delivered at your premises or at the Perugia and Città della Pieve clinics:

Workplace first aid (Italian Ministerial Decree 388/2003)

First-aid officer training with certified instructors, in collaboration with the centre’s Training division.

Training on specific risks

Video terminals, manual handling, chemical agents: modules built on the company’s actual risks.

Ergonomics and postural exercise

Practical sessions to reduce musculoskeletal complaints in sedentary and repetitive roles.

Nutrition and healthy lifestyle

Seminars on diet and healthy habits for employees.

Privacy-first reporting

Every visit is recorded in a GDPR-compliant digital health record, with two levels of output: the individual report given to the worker, and an aggregated, anonymous report for the company, allowing HR and safety officers to monitor overall trends without accessing individual data.

Tax-deductible costs

The mandatory costs of occupational medicine are fully deductible as business expenses (art. 95 of the Italian tax code), while additional healthcare services offered to employees may fall under corporate welfare (art. 51). For specific tax matters, please consult your advisor.
